SMS Terms & Conditions

These terms govern the text messaging program of PB Health & Wellness, A Naturopathic Corporation, doing business as The Nook Integrative Health ("The Nook").
Program description. The Nook Integrative Health Patient Notifications sends appointment reminders and confirmations, scheduling changes, and administrative and billing notices, such as reminders to complete intake forms or update a card on file. We do not send marketing or promotional messages.
Opt-in. You join by answering “Yes” to the text message consent question on our intake form and providing your mobile number. Consent to receive text messages is not a condition of purchasing any service or receiving care.
Message frequency. Message frequency varies based on your appointments and account activity.
Rates. Message and data rates may apply. Check your mobile plan for details.
Opt-out. Reply STOP (or CANCEL, END, QUIT, or UNSUBSCRIBE) to any message to cancel. You will receive one final message confirming you have been unsubscribed, and no further messages will be sent. To rejoin, reply UNSTOP or opt in again on our form. You may also opt out by emailing hello@thenooksf.com or calling (415) 255-5510.

Carrier liability. Carriers are not liable for delayed or undelivered messages.
Eligibility. You must be 18 or older, or the parent or legal guardian of a patient, and the account holder or an authorized user of the mobile number you provide.
Emergencies. This program is not monitored for emergencies. For a medical emergency, call 911.
Content. We do not send illegal, misleading, deceptive, or harmful content through this program.
Compliance. We operate this program in accordance with the Telephone Consumer Protection Act (TCPA), the CTIA Messaging Principles and Best Practices, mobile carrier guidelines, and applicable privacy laws, including HIPAA.
Privacy. Your information is handled as described in our Privacy Policy. No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
Changes. We may update these terms and will post the current version on this page.
